In February 2018, St. John the Baptist Parish began its Senior Care Program, which we are excited to offer! As a Parish with a significant aging population, so many of our parishioners and their loved ones are sometimes confronted with: rehabilitation after illness; requiring assistance to remain safely in their homes, and; possible long-term care placement. This can be a frightening and exhausting experience.
St. John the Baptist is the eleventh Parish in the Archdiocese of Philadelphia to offer this Program, in partnership with Catholic Housing and Community Services and partially funded by a generous endowment.
Jennifer Rose, MSW, LSW, is in charge of the Senior Care Program at St. John the Baptist. Examples of services Jennifer provides include: exploring and applying for benefits for which seniors may qualify; assessing the home situation for safety and making recommendations; connecting seniors to resources in the community to set them up for success in continuing to live at home, and; navigating Medicare and discussing which options best suit each senior’s individual needs. Jennifer has worked with seniors since 2005 in a variety of capacities. She has been in her current position for over two years.
The primary mission of the Senior Care Program is to help seniors safely remain in their homes and/or to support the transition to and from home, hospital, skilled nursing facility, or long-term care. By providing information and in-home evaluations, a plan is tailored for the needs and preferences of the senior, couple, or loved ones.
Services offered through the Senior Care Program:
· Selecting a new Prescription Drug Plan (Medicare Part D) or
Medicare Advantage Plan (HMO or PPO)
· Coordinating home care services
· Explaining the difference between home health care and
· Applying for VA benefits, such as Aid & Attendance
· Applying for benefits offered by Pennsylvania, such as
Rent/Property Tax Rebate, SNAP (formerly called food stamps),
LIHEAP (utility assistance), and Waiver (home care)
Coordinating legal assistance to create or update POA, will,
and advance directive documents
